A 57.57 mile long route, State Road 21 joins Jacksonville with the Clay County suburbs, Middleburg, Keystone Heights and SR 20 between Hawthorne and Interlachen. Through Putnam and the southern notch of Bradford County, SR 21 constitutes a rural two lane highway. Navigating along a winding course north of Melrose, SR 21 passes between Santa Fe Lake and Lake Geneva into the city of Keystone Heights. Angling northeast from SR 100, SR 21 crosses Lake Brooklyn ahead of Gold Head Branch State Park and Camp Blanding Joint Training Center.

SR 21 intersects SR 16 west of Penney Farms. Continuing north, SR 21 runs along the east side of the Satsuma Tract of Belmore State Forest to Blanding Boulevard and outer reaches of Middleburg. Blanding Boulevard constitutes a major commuting corridor northward to Lakeside, Bellair-Meadowbrook Terrace and Interstate 295 at Arygle Forest in Jacksonville. Within the Jacksonville Beltway, SR 21 (Blanding Boulevard) extends to U.S. 17 (Roosevelt Boulevard) and Park Street at Avondale.
