U.S. 158 originates in Mocksville and concludes along the Outer Banks at Whalebone. Initially paralleling Interstate 40, U.S. 158 heads northeast into Winston-Salem, where it combines with U.S. 421 along Salem Parkway south of Downtown. Shifting north, U.S. 158 bypasses Greensboro through Stokesdale en route to Reidsville in Rockingham County.

Navigating along a rural course, U.S. 158 continues east to the county seats of Yanceyville, Roxboro and Oxford. U.S. 158 combines with I-85 along the north side of Henderson and then with U.S. 1 east to Norlina. Within Northhampton County, U.S. 158 joins the city of Roanoke Rapids with I-95 and the seat at Jackson.

East from an overlap with U.S. 13 in Gates County, U.S. 158 enters Pasquotank County, where it combines with U.S. 17 southeast into Elizabeth City. Approaching the Outer Banks, U.S. 158 crosses Camden County to MSR 168 at Barco in Currituck County. The four lane highway turns southeast along a peninsula between Albemarle and Currituck Sounds to the Wright Brothers Memorial Bridge to Kitty Hawk in coastal Dare County.
