U.S. 78 overlaps with U.S. 278 across Atlanta, exiting the city along Ponce de Leon Avenue through Druid Hills. This stretch includes concurrences with U.S. 29 east from Northside Drive and U.S. 23 from Moreland Avenue. Through Decatur, U.S. 29/78 arcs north, avoiding Downtown along Scott Boulevard, a four lane boulevard north from Ponce de Leon Avenue to Church Street (old U.S. 29 Business). The route was proposed to bypass Decatur and North Decatur along an extension of the Stone Mountain Freeway. The American Association of State Highway Officials (AASHO) approved the realignment of U.S. 78 onto the unbuilt facility on October 26, 1969. The freeway was cancelled west of the separation with U.S. 29 (Lawrenceville Highway), and completed by late 1969 from there to Interstate 285 (Perimeter Highway). The remainder of Stone Mountain Freeway replaced Memorial Highway (SR 10) as the route of U.S. 78.

Through Athens, U.S. 78 shifted southward onto the Athens Perimeter Highway (SR 10 Loop) in 1988. The former surface route through Downtown was redesignated as U.S. 78 Business. Both changes were approved by the American Association of State Highway and Transportation Officials (AASHTO) on June 7, 1988.

U.S. 78 bypasses Washington to the north along a 4.7 mile loop to the north of the Wilkes County seat. With the new alignment of U.S. 78, the former route through Downtown was redesignated as U.S. 78 Business. AASHTO approved the Georgia application for both changes on June 30, 1970.
