U.S. 11 parallels Interstate 81 northward from Pennsylvania at Kirkwood to Binghamton, Cortland, Syracuse and Watertown. The two routes meet several times, with U.S. 11 generally serving local traffic. Leading away from Watertown, U.S. 11 connects with I-781 by Fort Drum before leaving the I-81 corridor en route to Gouverneur, Canton and Potsdam. The remainder of the route runs east to Malone, I-87 at Champlain and Lake Champlain at Rouses Point. U.S. 11 concludes at the Canadian border north of U.S. 2 and Rouses Point.

NY 222 begins at the intersection of Church Street and Clinton Avenue, where U.S. 11/NY 41 split with NY 13 in Downtown Cortland. NY 222 follows U.S. 11/NY 41 (Clinton Avenue) west to Groton Avenue. The first NY 222 shield was posted where U.S. 11/NY 41 turn north onto Main Street. 05/07/05
NY 9B (Lake Street) north ends at U.S. 11 (Champlain Street) in the village of Rouses Point. U.S. 11 overtakes Lake Street north from the state route to U.S. 2 (Bridge Road). 07/22/00
