NC 103 is a multi state highway with SR 103 in Virginia between Mount Airy and SR 8 near Five Forks. NC 103 stems east from U.S. 52 Business/SR 89 (Renfro Street) at Downtown Mount Airy along E Pine Street. Pine Street leads NC 103 to Flat Rock and Slate Mountain.

SR 1748 along NE Pine Street represents an old alignment of NC 103 near Faulkner Creek. NC 103 turns northeast from SR 1748 between Slate Mountain and Chestnut Ridge to the state line at Silverleaf Creek.

Virginia Route 103 extends 11.43 miles east into Patrick County. SR 103 travels to Claudville south of Simmons Mountain and Peters Creek south of Carter Mountain.
