Consisting of two state-maintained and three county-maintained sections, the road system that comprises SR 865 and CR 865 makes it the longest designated route in Lee County. Traveling a total of 39.5 miles from the eastern reaches of Fort Myers to Fort Myers Beach and Bonita Springs, only 5.9 miles are designated as a state road.
Lee County Road 865 originates in Tice at SR 80 (Palm Beach Boulevard) opposite Schandler Hall Community Park and travels south on Ortiz Avenue through eastern reaches of Fort Myers. Six Mile Cypress Parkway continues CR 865 beyond the intersection with SR 884 (Colonial Boulevard) on a southwesterly course through Six Mile Cypress Slough Preserve to Daniels Parkway (CR 876). The county road continues south and west to meet SR 739 (Metro Parkway) opposite SR 865.
The first instance of Florida State Road 865 beelines west from Metro Parkway on a 1.1 mile course to U.S. 41 (Tamiami Trail) and Gladiolus Drive in South Fort Myers.
CR 865 resumes beyond U.S. 41 and overlays Gladiolus Drive westward along a six lane arterial around Lakes Park to Summerlin Road (CR 869) and Harlem Heights. The 4.8 mile stretch of county road concludes at SR 867 / CR 867 (McGregor Boulevard) and SR 865 (San Carlos Boulevard) at Miners Corner.
The second iteration of SR 865 extends south along San Carlos Boulevard 4.8 miles to Fort Myers Beach. This segment of the state road ends in the heart of Fort Myers Beach at Estero Boulevard (CR 865) at the south end of the Matanzas Pass Bridge.
CR 865 continues from the end of SR 865 along Estero Boulevard on a southeasterly course on the Gulf side of Estero Island to Big Island Pass and Lovers Key State Park. Beyond the state park, CR 865 overtakes Hickory Boulevard southeast through Little Hickory Island before turning inland for Bonita Springs. The county road overlays Bonita Beach Road for its remainder through central Bonita Springs before meeting I-75 at a diamond interchange. While CR 865 ends at I-75, Bonita Beach Road continues 5.1 miles east to end opposite the Bonita National subdivision.
