U.S. 258 originates in Jacksonville in southeastern North Carolina. Following a 155 mile course within the Tar Heel State, U.S. 258 runs north to Kinston, Farmville, Tarboro and Scotland Neck. The highway enters Virginia north of Murphreesboro.

Within Pitt County, U.S. 258 was bypasses the town of Farmville. Approved by the American Association of State Highway and Transportation Officials (AASHTO) on October 23, 2009, the realignment of U.S. 258 follows U.S. 264/Future I-587 east 2.96 miles from their exchange north of Little Contentea Creek to Wesley Church Road (SR 1221). U.S. 258 overtook Wesley Church Road from SR 1221 south 2.49 miles to U.S. 264 Alternate. U.S. 258 and U.S. 264 ALT overlap 0.47 miles west to U.S. 258 Business along S Main Street. The former alignment, 3.23 miles north to Downtown Farmville along Main Street, Wilson Street and May Boulevard was redesignated as U.S. 258 Business.
