U.S. 23 travels north from the Grant Bridge crossing the Ohio River at Portsmouth to the Michigan state line alongside U.S. 223 at Sylvania. Through southern Ohio, U.S. 23 joins the County seats of Waverly, Chillicothe and Circleville. U.S. 23 overlaps with U.S. 35/50 along a freeway bypassing Chilliocthe to the east as well.

Through the capital city of Columbus, U.S. 23 follows High Street north to Downtown and the one-way couplet of 4th Street north and Summit Street south to the east of Ohio State University. Paralleling I-71, U.S. 23 continues along Indianola Avenue to Morse Road west. U.S. 23 rejoins High Street through Worthington and north Columbus.

Advancing into northern Ohio, U.S. 23 is a four lane corridor with expressway bypasses of Delaware, Marion and Upper Sandusky. The four lane corridor branches west along U.S. 68 to Findlay, while U.S. 23 continues north to Fostoria and U.S. 20 along the Wood and Sandusky County line.

U.S. 23 overlaps with U.S. 20 west to Perrysburg, where it joins I-75 south and I-475 north to bypass Toledo. I-475/U.S. 23 part ways near Sylvania, where U.S. 23 continues along a freeway into Michigan.
