Hawaii Route 99 follows the western third of Kamehameha Highway from I-H1/Route 92 by Joint Base Pearl Harbor-Hickham (PHH) north to Route 83 (Joseph P. Leong Highway) near Hale'iwa on Oahu's North Shore. An exception occurs between Pearl City and Waipahu, where Route 99 shifts onto Farrington Highway due to the original construction of I-H1/H2. Route 99 also diverges from Kamehameha Highway along Wilikina Drive and Kamananui Road to circumvent Wahiawa west to Schofield Barracks.

Route 99 (Kamehameha Highway) branches north from the exchange joining I-H1 with Route 92 (Nimitz Highway) west into Joint Base PHH. The four lane highway runs north to Pearl Harbor and Aloha Stadium. Route 99 turns west at the exchange with Route 78 (Moanaula Road) at Aiea. Running north of the Pearl Harbor East Loch, Kamehameha Highway continues adjacent to the Skyline light rail line to Waimalu and Pearl City.

Kamehameha Highway shifts north of parallel I-H1 along a commercial arterial west across Pearl City. Route 99 separates from Kamehameha Highway for the eastern extent of Farrington Highway at a wye interchange older than Hawaii's statehood. Historically this is where Route 99 began and Route 90 shifted from Kamehameha Highway west onto Farrington Highway to Waianae. Kamehameha Highway southeast into Honolulu was also originally part of Route 90.

The section of Route 99 on Farrington Highway links the discontinuous sections of Kamehameha Highway at Waipahu and Pearl City. The Kamehameha Highway spur west from Route 99 in Pearl City connects with Waihona Street and ramps with I-H1/H2. Route 99 rejoins Kamehameha Highway at a trumpet interchange adjacent to Leeward Community College.

Kamehameha Highway parallels I-H2 (Veteran's Memorial Freeway) to the west through the communties of Waipi'o, Mililani and Waipi'o Acres. The two routes meet by Wheeler Army Airfield. Route 99 turns west onto Wilikina Drive to overtake the north end of I-H2 while Kamehameha Highway continues north as Route 80 into Wahiawa.

Wilikina Drive encircles Wahiawa Reservoir west to Schofield Barracks along a 1.82 mile long course from I-H2 to Kamananui Road. Wilikina Drive extends another 5.82 miles as Route 803 to Farrington Highway (Route 930) while Route 99 turns northeast along Kamananui Road for 1.18 miles to rejoin Kamehameha Highway (Route 80). Route 99 concludes at a shared endpoint with Route 83 (Joseph P. Leong Highway) and Route 930 along the continuation of Kamehameha Highway into Hale'iwa.

Originally Route 99 traveled northwest along Kamehameha Highway from Route 90 and Farrington Highway in Pearl City to Wilikina Drive by Wahiawa. Route 99 then followed all of Wilikina Drive northwest from Schofield Barracks back to Farrington Highway near Waialua. Route 99 was the designation for Farrington Highway west to Mokule'ia and along the unpaved section to Ka'ena Point.
