U.S. 15 enters North Carolina coupled with U.S. 401 into Scotland County. The U.S. 15/401 overlap stretches 34 miles from Society Hill, South Carolina to Laurinburg. U.S. 501 join the pair in Laurinburg along an older bypass of Downtown. North from the city, U.S. 401 branches easterly toward Raeford and Fayetteville while U.S. 501 remains alongside U.S. 15 north to Southern Pines.

Beyond a short overlap with U.S. 1, U.S. 15/501 continue along a shared alignment north toward Carthage before converging with U.S. 1 eastward again in Lee County. U.S. 1-15-501 overlap into the city of Sanford, with Downtown served by a Business loop for U.S. 1. U.S. 15/501 split with U.S. 1 alongside NC 87, from north Sanford to Pittsboro.

North from U.S. 64, U.S. 15/501 expand into a four lane highway from the outskirts of Pittsboro into Chapel Hill and the city of Durham. Once in the Bull City, U.S. 15/501 shift to a freeway bypass north to I-85/U.S. 70. I-85 carries all three routes east three miles to Duke Street, where U.S. 501 parts ways with U.S. 15 for Roxboro. U.S. 70 follows suit in another two miles for Raleigh while U.S. 15 remains paired with I-85 northeast to Granville County outside Butner. U.S. 15 intertwines with Interstate 85 northward to Creedmoor and Oxford before leaving the state to Clarksville, Virginia.

NC 751 enters the city of Durham from the south via Hope Valley Road. It moves onto Academy Road near U.S. 15-501 Business, then changes into Cameron Boulevard by Duke University. This shield assembly precedes the U.S. 15-501 freeway along northbound NC 751 (Cameron Boulevard). 06/05/05
A right turn from northbound NC 751 (Cameron Boulevard) connects to U.S. 15-501 north. The next four exits on northbound U.S. 15-501 are Morreene Road, NC 147 (Durham Freeway), U.S. 70 Business (Hillsborough Road) and I-85/U.S. 70. 06/05/05
